Janumet XR Dosage and Administration
- The suggested beginning dose in those not currently treated with metformin is 100 mg sitagliptin and 1000 mg metformin HCl once daily, with a gradual increase in dose to reduce the gastrointestinal side effects associated with metformin.
- The initial dose in patients already taking metformin should provide sitagliptin dosed as 100 mg and the metformin dose already being taken once a day. For those taking metformin HCl 850 mg or 1000 mg twice daily, the suggested starting dose of JANUMET XR is two 50 mg sitagliptin and 1000 mg metformin HCl extended-release tablets once a day.
Your healthcare provider may start you on a dose lower than what is expected in the beginning to test your tolerance to the drug. They may then gradually increase your dose until your blood glucose is adequately controlled. Gradually increasing your dose can also help reduce side effects.

Contraindications
